Councilmember Teresa Mosqueda hosts a lunch and learn with Transform Health to discuss how the City of Sacramento is building an integrated system of care for the most vulnerable of the homeless population by bringing together health and housing partners. Through a health and equity lens, the city is helping stabilize the lives of the chronically homeless who are high utilizers of health and EMS services by addressing their healthcare, housing and social service needs to reduce barriers to accessing and maintaining housing.
